xi. Current Tax Clearance for the last three years (201 – 2015)
xii. List of Equipment for the proposed project where necessary
xiii. Name, e-mail address and telephone numbers of contact persons for the project
xiv. Affidavit of disclosure that none of the persons connected with the bid process in the procuring entity has any pecuniary interest and that the company is in receivership or any form of insolvency/bankruptcy nor in debarment and that the company nor any of the Director(s) of the company have been convicted of financial crime
xv. Verifiable office address in Abuja

ADDITIONAL MANDATORY REQUIREMENTS FOR THE PROVISION OF INSURANCE
i. Published Annual Reports and Accounts approved and recently certified by the National Insurance Commission for the last 3 financial years – 2013, 2014 and 2015
ii. Certified True Copy of the Company’s registration as an Insurer issued by the National Insurance Commission (NAICOM) recently certified by the Commission
iii. Evidence of certification with relevant provisions of the NSITF Act
iv. Evidence of Registration with BPP
v. NAICOM’s approved annual returns and account for the underwriting year ending 31st December, 2016
vi. Any other information that may show the broker’s ability to carry out the assignment satisfactorily

IMPORTANT INFORMATION
i. All documents submitted will be subjected to verification from the relevant agencies
ii. Only responsive Technical Bids submitted will be subjected for detail evaluation
iii. NGCOMSAT Limited reserve the right to reject any Bid Document, and is not bound to accept the lowest Bid or any Tender
iv. NIGCOMSAT Limited reserves the right to annul the bidding process at any time without incurring any liabilities and assigning reason thereof.

SUBMISSION OF TENDER DOCUMENT
Completed Bids must be submitted on or before 12.00noon on Monday, 17th October, 2016 in two separate sealed envelopes and marked “TECHNICAL BID FOR LOT…” or “FINANCIAL BID FOR LOT…” at the top left hand corner of each envelope with the Company name indicated at the back of each envelope. The Nigerian Airspace Management Agency (NAMA), was established by Act No. 48 of 1999 with a mandate to develop the Nigerian Airspace to a level consistent with the requirements of the International Civil Aviation Organization’s (ICAO) Standards and Recommended Practices. C. PREQUALIFICATION AND EXPRESSION OF INTEREST REQUIREMENTS
To qualify for consideration, interested companies with experience in similar jobs are required to submit the documents listed hereunder alongside their Prequalification and Expression of Interest documents. Failure to submit any of the underlisted documents may lead to disqualification of the contractor/supplier/consultant.
1. Evidence of Incorporation with Corporate Affairs Commission (CAC) of Nigeria
2. Certified true copy of Memorandum and Article of Association with CAC (Form C02 & CO7 inclusive)
3. Tax Clearance Certificate (TCC) for the last three (3) years (2013 – 2015) corresponding with declared Annual Turnovers by inclusion of photocopy and Tax identification Number (TIN) issued by the proper tax authority
4. Evidence of VAT registration and proof of past remittances
5. National Pension Commission (PENCOM) Certificate as evidence of compliance with Pension Reform Act 2004
6. Industrial Training Fund (ITF) Certificate as evidence of compliance with the Industrial Training Fund Amendment Act, 2011
7. Evidence of registration on the National Database of Federal Contractors, Consultants and Service Providers (Interim Registration Report from the Bureau of Public Procurement)
8. Detailed Company Profile and Professional experience/Curriculum Vitae of key personnel, including registered address, name, functional location, Telephone Nos and Email Address etc
9. Audited statements of accounts for the past three (3) years (2013 – 2015)
10. Verifiable evidence of competence, exhibited in the provision of similar services within the last 3 years, Attach letters of award and certificate of disengagement if any
11. Evidence of membership of Relevant Professional bodies or Government Agency (where it is deemed necessary)
12. Letter of Recommendation from Company’s Bankers indicating the Financial capacity and capability to undertake the project if awarded
13. Evidence of ownership or access/lease of relevant equipment/technology and resources required to be deployed for the project
14. Evidence of dealership or manufacturers authorization (where it is deemed necessary) & any additional information that may enhance the pre-qualification of the company
15. For Joint Ventures (JVs), Tender to include Memorandum of Understanding (MOU) (indicating the responsibilities and duties of the individual firms constituting the JV. The lead Consultant – the firm which has the most relevant core competence for the Assignment, must be indicated in the Consortium’s MOU.
16. Sworn affidavit disclosing as follows:
• That all documents submitted are not only genuine but correct
• That the Director(s) has/have never been convicted by any Court of law
• That the firm’s Director or Company is not bankrupt
• That the Company is not a replacement for a hitherto tax defaulting company
• That the company is not in receivership or financially/legally encumbered.
• Whether or not any of the Officers of NAMA or Bureau of Public Procurement (BPP) or Federal Ministry of Transportation is a former ot present Director of the company

D. ADDITIONAL REQUIREMENTS
a) Companies submitting Expression of Interest for Group Life Insurance, shall in addition to the requirements stated above also possess the following:
i. Evidence of Registration and Certification with National Insurance Commission (NAICOM)
ii. Evidence of Registration with National Insurance Association (NIA)
b) Companies submitting Expression of Interest (EOI) for Social Health Insurance Scheme shall in addition to the requirements listed earlier also submit the following
i. Registration and Certification as a Health Maintenance Organization (HMO) by the National Health Insurance Scheme (NHIS) and the Federal Ministry of Health

2. ELIGIBILITY CRITERIA:
Interested contractors are to submit the following documents in the order stipulated below:
(i) Evidence of incorporation and registration with CAC
(ii) Genuine Tax Clearance for the proceeding 3 years i.e. 2013, 2014 and 2015
(iii) Evidence of current pension clearance certificate from PENCOM(for year 2015)
(iv) Evidence of compliance certificate with the provision of section 6(1) (3) of the amended Industrial Training Fund (ITF) Act No. 19 of 2011/Payment of Training Contributions to the Industrial Training Fund
(v) Certificate of Compliance with NSITF
(vi) Companies audited accounts for the proceeding 3 years, 2013, 2014 and 2015.
(vii) Evidence of Registration on the BPP’s National Database of Contractors, Consultants and Service Providers
(viii) Profile of the Company including list of similar projects executed, Letter of Awards/completion certificates and curriculum vitae of professionals in the Company’s employment backed by Certificate
(ix) Evidence of financial capability supported with bank statements for the last six months from a reputable bank
(x) Interested Firms MUST NOT submit bids for more than tow (2) lots
(xi) Every submission must be accompanied with an affidavit that all information and documents submitted are true and correct and that no officer of the Commission is a former or present director, shareholder or has any pecuniary interest in the bidder.
(xii) Please note that copies of Tax Clearance and Pencom Certificates will be forwarded to relevant Government Authorities for validation by the Commission in the course of the bidding process.
(xiii) Please note that for: FMEC-1, AD-12, AD-13, IT-9, Sa-2, AD-7, CM-1, SA-1, SA-2, DB-16, ICT-1 and SCR-1. Bidders must have annual turnover of N100,000,000.00 and above.
(xi) Please note that for: AD-10, FS-2, TS-5, DB-1, DB-15, Dap-1, ADp-1, WIC-1 and ECC-1, bidders must have annual turnover of N200,000,000.00 and above

3. COLLECTION OF REQUEST FOR PROPOSAL/INVITATION TO TENDER
The collection of RFP attracts a tender fee of N20,000 payable through the Federal Government Treasury Single Account in favour of Nigeria Communication Commission. Evidence of payment of the tender fee must be attached when returning the RFP.

4. The prospective bidders shall enclose the technical and financial bids in two (2) separate envelopes, and enclose the two envelopes in one sealed outer envelope clearly marked “REQUEST FOR PROPOSAL – LOT No…”/”INVITATION TO TENDER-LOT No….”(as the case may be) on the top right hand corner which shall bear the lot description, the name and address of the company on the reverse side of the envelope and addressed to:

3.0 ELIGIBILITY
(i) Evidence of registration with Corporate Affairs Commission (CAC) by inclusion of Certificate of Incorporation
(ii) Evidence of compliance with Federal Inland Revenue Service by inclusion of Tax Clearance Certificate for the last three years (2015, 2014 and 2015)
(iii) Evidence of compliance with the National Pension Commission (PENCOM) Act 2014 by inclusion of compliance Certificate
(iv) Evidence of compliance with the Industrial Training Fund Act 2011 by inclusion of Compliance Certificate
(v) Evidence of registration with Bureau of Public Procurement (BPP) on the National Database of Contractors, Consultants and Service Providers
(vi) Evidence of financial capability supported with Bank reference and commitment from a reputable bank;
(vii) Detailed Company profile with curriculum vitae of key officers/Professional Personnel including photocopies of relevant Professional Qualification
(viii) Verifiable evidence (copies of letter of awards and completion certificates) of successful completion of similar jobs in the last five (5) years
(ix) Suppliers of Equipment/Motor Vehicles must either be the Manufacturers or accredited representative of the equipment manufacturer. Suppliers are to submit certificate of attestation.

4.0 SUBMISSION OF PRE-QUALIFICATION DOCUMENTS
Pre-qualification documents must be appropriately prepaid in hard copy and sent in a sealed envelope marked “PREQUALIFICATION FOR TENDER” and must be delivered to CGC Conference Room, Left Wing, 2nd Floor, Nigeria Customs Service Headquarters, No. 3 Adidjan Street, Wuse Zone 3, Abuja.

The document shall be deposited in tamper proof box which shall be supplied for this purpose.

5.0 SUBMISSION OF DEADLINE
All submission must be made on or before 4.00pm Monday, 1st August, 2016.
